Licensed Independent Practitioners (LIP) that would like to apply for clinical privileges at Magnolia Hospital should complete a

pre-application and submit to the Credentialing Office.  A pre-application can be downloaded here.  When submitting a pre-application, all fields should be completed, and the pre-application should be signed.

 

Magnolia Hospital requires the following from all LIP with privileges at our facility:

· Currently valid license issued by the State of Arkansas in your appropriate field

· Currently valid DEA registration certificate

· Graduation from an approved Medical School (appropriate to your field)

· Completion of a residency program approved by the Accreditation Council for Graduate Medical Education (MD’s only)

· Professional liability insurance with a licensed and approved carrier of $1,000,000/$3,000,000

 

All above information will be verified through the Arkansas Centralized Credentials Verification Services (for MD/DO/RT only)

 

In addition, applicants will be asked to submit a recent TB skin test (or negative chest x-ray), a list of professional peer references, and a recent photo.

 

Applicants should allow at least ninety (90) days for an application to be completely processed before privileges are granted.
